Established patients
If you are an existing patient, you may use the patient portal to request an appointment. Click Here to visit the patient portal or click the link in the main menu. You may also call our office and our reception staff will gladly assist you. Kindly give 24 hour notice if rescheduling or cancelling an existing appointment. For any questions or concerns please call and speak with our clinical staff, as they are happy to help you
New patients
We take great pride in the personalized care we provide each and every patient. To better coordinate your care, we require that all new patient requests be scheduled through physician referrals. As such, please contact your physician to begin this process.
Once your physician has referred you care to us, our scheduling coordinator will call you to get you registered and schedule your new patient appointment with you. They will then mail you a new patient packet for you to complete and bring with you to your 1st appointment. It is important that your referring physician supply us with all pertinent records so that your time here can be used to your best advantage. Please call your physician to ensure that your records have been sent to us.
To respect the appointment times of all our patients, we ask that you arrive 20 minutes early and that you bring your completed packet of forms, your insurance card(s), any pharmaceutical copay assistance card(s) you may have, a current list of all medications you take, an insurance referral from your PCP, if required by your insurance, and a valid photo id. If, however, you do not have your new patient packet completed prior to arriving to your scheduled appointment, please arrive at least 30 minutes prior to your scheduled appointment time to be able to do so before your appointment time.
Message to ALL patients
We try our best to stay on schedule to minimize your waiting time. With the complicated disease state of many of our patients, it is not always possible to stay on time. We appreciate your understanding and patience under these circumstances.
Interpreters
We can provide an interpreter for your appointment if you are not comfortable speaking English. If needed, please contact our office as soon as possible as we need ample notice to arrange this service for you.
Consultations / Referrals
Prior Authorizations
There may be a need to acquire pre-authorization or pre-determination on some procedures depending on insurance requirements. We understand this process and will help in getting the approvals needed for you treatment. In the rare event that your insurance denies a procedure we will work with you financially to make this treatment possible.
Appointment Cancellations
If you cannot make it to your appointment, please contact us at least 24 hours in advance of your scheduled appointment time, and we will work with you to reschedule your appointment. In the event our office is closed, please leave a message in the general mailbox.
If you miss your appointment, are more than 5 minutes late for your appointment, or need to cancel or change your appointment with less than 24 business hours’ notice, you will be charged a $75 no show fee. We have this policy to respect our patients that are on a waiting list to be able to give them ample notice to arrive in a timely fashion for their appointment.
We do understand the need for emergency cancellations. Feel free to contact our Billing Manager, Annamarie, at extension 1235 if you need to discuss a missed appointment invoice.
We hope you understand that this policy is enforced to help protect the rights of our patients who need an appointment. This could be you one day.
